First thing I did was to have the pre-registration online so that when I arrive, it will be less hassle and less time at the reception area. We arrived at St. Lukes at around 4:30am last August 9, 2011. There were already some people who were in the line, so we also fall in line. The guard let us entered the clinic at around 5:30am.
In my experience, I only brought with me the following:
· Pre-registration form (online) (2 copies)
· Scanned copy of my NOA2 (I photocopied it)
· Passport
· Photo copy of my passport
· 3 pieces 2x2 picture
· And lastly, I took down notes of my MNL Case Number, I got it thru phone.

I handed over to the guard my Pre-registration form, Scanned copy of my NOA2 , Passport, Photo copy of my passport, 3 pieces 2x2 picture. Then he checked it and he only took my 1 copy of the pre-registration form and he inserted a red paper on the top of the form that he gave me. I entered to the clinic and filled up those boxes that the guard instructed.
First Day:
· 1st. Submit the form (that the guard handed you) immediately at the window that says “red, yellow and green” when you pre-registered online. But if you don’t have that registration, the guard will give you a number and the form and wait for your number to be called.
· 2nd . Payment at 2nd floor.
· 3rd. X-ray at 3rd floor.
· 4th. Urine test.
· 5th. Physical Examination.
a. Weight, height, eye test, vital signs, etc.
b. The assigned physician will ask you things like: any major operation? Do you smoke? Expose to a person with TB? Infection? Vehicular accident? When your first and last period? Chicken pox? And many more. Then she will let you take off your clothes (ALL) to check your breast and your butt. (mine was butt only)
· 6th. Proceed to the 2nd floor for the Immunization Interview with the Physician assigned. It will be the same question during your physical exam just always bear in mind your first day and last day of your period. This is also in this stage that the Physician will ask you if you’re ready for your shots (vaccines) on the next day. And the staff will write on your receipt on what time your result will be released.
Second Day.
We arrived 9am on the second day, we handed our passport and receipt to the assigned guard. He entered the receipt number and it says “FOR IMMUNIZATION” Thank You Lord! We went to the 2nd floor. We passed our receipts and waited for an hour. When our name was called we entered to the Immunization room, the nurse (who had a bad mood) gave me the 3 shots! It was painful but it was okay! Then we proceed to the ground floor and waited for almost 3 hrs! LOL! The assigned personnel let me signed some papers then back to my seat again. After 30 minutes, my name was called for further instructions and interviews. Then he handed to me the xray CD, and Vaccination Documentation Worksheet. And we’re ready to eat at Robinsons Mall! LOL!
To make the story short, I PASSED THE MEDICAL Exam!I Love you soo much kokoleo na gwapo!
